Pregnancy Notification and Maternal and Child Health Handbook in Shinjuku City

When you become pregnant, submit a Pregnancy Notification (妊娠届) to receive a Maternal and Child Health Handbook (母子健康手帳) and the Mother and Child Health Bag. Foreign-language editions (10 languages) are provided as reference material, only to foreign-national expectant mothers who do not understand Japanese and ask for one.

- Special Branch Offices (Pregnancy notification accepted, but Yurikago Shinjuku interview is not conducted)
What to Bring(1) Pregnant woman's Individual Number document (My Number Card, notification card, etc.)
(2) Submitter's identification document (Residence Card, My Number Card, passport, driver's license, etc.)
(3) Power of attorney (委任状) if someone outside your household submits the notification for you.
*Be ready to write the weeks of pregnancy, the expected delivery date, and the name and address of the clinic that diagnosed your pregnancy. This page does not recommend any specific hospital or clinic.*
Foreign-Language HandbooksProvided as reference material (1 copy per person) only to foreign-national expectant mothers who do not understand Japanese and ask for one.

- Special Branch Offices: English, Korean, and Chinese only.

Online ApplicationYou can submit the notification through Mynaportal (Pittari Service), but the procedure is not complete until you pick up the handbook. Contact the Health Promotion Division (03-5273-3047) to arrange the date and place of pickup at a Health Center or the division. It cannot be sent by mail. If you want to receive the handbook at a Special Branch Office, do not apply online; submit the notification at the counter.

  • <b>外国語がいこくごの手帳てちょう</b>：日本語にほんごが分わからない外国籍がいこくせきの妊婦にんぷで、希望きぼうする人ひとに、参考さんこうとして1人ひとり1冊さつ渡わたします。保健ほけんセンターは10の言葉ことば（英語えいご・中国語ちゅうごくご・韓国語かんこくご・タイ語ご・タガログ語ご・ポルトガル語ご・インドネシア語ご・スペイン語ご・ベトナム語ご・ネパール語ご）、特別出張所とくべつしゅっちょうじょは英語えいご・中国語ちゅうごくご・韓国語かんこくごのみです。夫おっとだけが外国籍がいこくせきのときは対象たいしょうになりません。
  • <b>オンライン申請しんせい</b>：マイナポータル（ぴったりサービス）から出だせます。でも、手帳てちょうを受うけ取とるまで手続てつづきは終おわりません。郵送ゆうそうではもらえないので、健康けんこうづくり課か（03-5273-3047）へ電話でんわして受うけ取とる場所ばしょと日時にちじを決きめます。特別出張所とくべつしゅっちょうじょで受うけ取とりたい人ひとは、オンラインで出ださないで窓口まどぐちで届とどけ出でます。
