Child Medical Expense Subsidies in Shinjuku City

In Shinjuku City, child medical expense subsidies cover the out-of-pocket copayment for health insurance-covered treatments and in-hospital meal expenses for children. There is no income limit for the guardian. This page does not recommend any specific hospital or clinic.

Who it is for

Children registered as residents in Shinjuku City until the first March 31 after reaching age 18, and guardians raising them.

What to do

Steps: when, where, and what to bring
Three Certificates(1) Infant Medical Certificate (マル乳): for preschool children, until the first March 31 after the child turns 6
(2) Child Medical Certificate (マル子): for elementary and junior high school students, until the first March 31 after the child turns 15
(3) High School Student Medical Certificate (マル青): until the first March 31 after the child turns 18 (whether or not the child attends high school)
How to UseShow the medical certificate together with your My Number Card (used as a health insurance card) or Eligibility Confirmation Letter at a contracted medical institution in Tokyo. Bring the certificate even when you use the My Number Card. The certificate is renewed automatically every October 1; you do not need to apply for renewal.
When to ApplyIf you apply within 3 months from the day after the birth or the move into Shinjuku City, the subsidy starts from the day of birth or move-in. If you apply later, it starts from the 1st day of the month you apply, and it cannot go back further.
What to Bring- Application form (乳幼児・子ども・高校生等医療証交付申請書)
- A document that shows the child's health insurance (e.g., Eligibility Confirmation Letter). The old paper insurance card expired on December 1, 2025.
- A document that shows the child's My Number (My Number Card or notification card)
- Your ID (My Number Card, driver's license, etc.) — only if you come to the office
Where to ApplyChild Medical and Allowance Section (子ども医療・手当係), Child Development Division (児童育成担当課), Shinjuku City Office Main Building, 2F, Counter 15, or a Special Branch Office (特別出張所). You can also apply by mail or online through Mynaportal (ぴったりサービス).
Outside TokyoApply for the "medical expense subsidy" (医療費の助成申請) if you paid the hospital meal expense, received care outside Tokyo, or paid the copayment because you could not show the certificate. Bring the original receipt (a receipt with only the amount is not accepted).

  • <b>いつまでに</b>：生うまれた日ひや新宿区しんじゅくくに引ひっ越こした日ひの次つぎの日ひから3か月げつ以内いないに出だすと、生うまれた日ひや引ひっ越こした日ひから助成じょせいが始はじまります。遅おくれて出だすと、出だした月つきの1日ついたちから始はじまります。それより前まえには戻もどれません。
  • <b>持もち物もの</b>：申請書しんせいしょ、子こどもの健康保険けんこうほけんがわかるもの（紙かみの保険証ほけんしょうは2025年ねん12月がつ1日ついたちで使つかえなくなりました）、子こどものマイナンバーがわかるもの、来くる人ひとの本人確認書類ほんにんかくにんしょるい（窓口まどぐちに来くるときだけ）。
  • <b>どこへ</b>：区役所くやくしょ本庁舎ほんちょうしゃ2階かい15番ばん窓口まどぐち（児童育成担当課じどういくせいたんとうかの子こども医療いりょう・手当係てあてがかり）、または特別出張所とくべつしゅっちょうじょ。郵送ゆうそうや、マイナポータル（ぴったりサービス）のオンラインでもできます。
  • <b>東京都とうきょうとの外そとで受診じゅしんしたときなど</b>：お金かねを払はらった後あとに、区役所くやくしょへ助成じょせい申請しんせいをします。入院にゅういんのときの食事しょくじ代だいを払はらったときや、医療証いりょうしょうを見みせられないでお金かねを払はらったときも同おなじです。領収書りょうしゅうしょの原本げんぽんを持もっていきます。
