| Where to apply | Child Medical Subsidy and Allowance Subsection (子ども医療・手当係), Child Development Section (児童育成担当課), Children and Family Department (子ども家庭部), Shinjuku City Office Main Building 2F, Counter 15; or any Branch Office (特別出張所). You can also apply by mail or online through Mynaportal (ぴったりサービス). If you apply by mail, the date of application is the day it arrives at the Child Medical Subsidy and Allowance Subsection, not the day you mail it, so send it early. | If your child lives somewhere else | You need the "Statement of Care for a Child Living Separately" (児童手当 別居監護の申立書) and a document showing the child's Individual Number. (If the child lives in Shinjuku City, you do not need the Individual Number document.) If your child is studying abroad, other documents are needed. Ask the Child Medical Subsidy and Allowance Subsection. |
