"Name" in Japanese is 名前 (namae). To ask someone's name politely, say お名前は何ですか or お名前をお伺いしてもよろしいですか.

Japanese names are usually family name first: 田中太郎 is Tanaka (family) Taro (given).
"My name is Smith" politely is スミスと申します.
